The cost of hospital readmission in the US has spiraled out of control. Research confirms that patients being readmitted to hospitals account for $15 billion in Medicaid and Medicare payments. The situation is alarming for uninsured Americans, particularly the elderly and the limited income group, who suffer from care-intensive and chronic diseases. For them, staying healthy can be outrageously costly because of expensive hospital readmissions and lack of health education. If they return, after being hospitalized for a condition and released, they are usually branded as “unplanned readmission”, which may add to their health care woes. A study conducted by CMS revealed that one in five patients admitted to hospitals for heart attacks, pneumonia and heart failure can return for treatment of the same condition even within a month's time. 

Hasty hospital readmission is an indicator of an inefficient and crumbling health care system. This is further aggravated by miscommunication between patients and doctors, insufficient discharge instructions as well as lack of beneficiary education and outpatient follow-ups. Americans living under the federal poverty line and suffering from chronic conditions, regularly put their health on hold because they cannot afford basic treatment or even the co-payment of a routine physical. Their return to the hospital, many of which is within 30 days, is fraught with financial difficulties and hardships. Sun Knowledge firmly believes that disease flare-ups and degradation can be combated through beneficiary education, clinician follow-up and meticulous adherence to drug regimens and post-hospital care plan.

To solve the re-admission riddle, Sun Knowledge has introduced a unique and dependable Remote Medical Home Service, emphasizing on patient’s empowerment and awareness about their own health conditions. Our qualified clinicians, supported by an elite team of American doctors, educate beneficiaries about their health conditions, while taking them through the fundamentals of their care plans. Our educative measures commence at the hospital, during the time of their release, followed by an array of integrated services including comprehensive reviews, follow-up visits as well as psychological and behavioral therapies. Our Remote Medical Home is a fairly complex process due to the number of interrelated processes, unique beneficiary profiles, and the task of truly integrating with a plan to take care management to the next level. 
 

Americans with low-incomes, chronic diseases and/or multiple prescriptions can avail of our Remote Medical Home Services.

Our simple and convenient process, befitting the needs of our client beneficiaries, is accomplished through five stages:
  • Initiating our Services at the Hospital: Our service begins at the time of a patient's release from hospital. One of our employees, present at the hospital site, educates the patient about his current health-condition and the care-plan he must stick to, while ensuring that the plan is imaged and sent across to our operations center. 
  • Primary Intervention and Education: Within 24-48 hours of our hospital visit, our experienced physician gets in touch with the beneficiary, patiently talking him through the intricate details of his care-plan. The goal of this stage is to educate the patient about the “do”s and “don’t”s of his care-plan, while explaining the medications and recommendations provided by his hospital. The promptness of our Primary Intervention services depends upon the intensity of the disease. During this interaction, our physician also gets to know, whether the beneficiary has filled in any prescription, provided by his hospital.  After the primary intervention call, he schedules a comprehensive medical review for his patient, while discussing his appointment with his primary care provider, if he has one. If our beneficiary is without a primary care provider, Sun Knowledge will render a host of concierge services to him, helping the patient to book an appointment with a particular health care provider.
  • Comprehensive Medical Review: Our expert physician, then, conducts a Comprehensive Medical Review with his patient, systematically collecting his information and assessing his medications and disease therapies. After addressing potential health problems, the doctor, subsequently, devises a plan to rectify them. By gauging the patient’s medication habits, he avoids administration of drugs that may lead to irreversible complications, underutilizations, allergies, conflicts and interactions. The motto of our physician is to make the patient aware of his medications, so that he can manage his own health-conditions and medications better. 
  • Follow-Up:  If warranted, our doctors even follow-up with the physicians of our beneficiaries, discussing effective ways to curb their potential health-risks and repeated disease flare-ups. The frequency of our follow-ups depends upon the seriousness and susceptibility of cases and how closely they need to be monitored in order to effect remission of problems.
  • Special Interest Group: Sun Knowledge also monitors specialty programs and is considering setting these up locally to assist members with education and cessation of smoking and drinking habits.
